[BLAST_SHIFTS] ion polarimeter HV info now in the MySQL database

Hi!
The ion polarimeter HV has been added to the MySQL database, as requested
by Adam, using voltages and other info provided by him and Yuan.
The table name is IP_HV (note there is another table POL_HV for Compton).
